Individuals in good health and doing normal activities with regular income
Age Range
Must be 6 months old to 59 years old at the time of application
Physical Exam Requirement
For applicants 51 years old and above, physical exam is required
Minor Applicants
For applicants who are minors, guardian must sign for the applicant
Up to 5 Dependents
Spouse, Child, or Parent
Qualified Dependents
Spouse (if married), children, and parents. Up to 5 dependents allowed
Child Dependents Requirements
Must be single, below 21 years old and not gainfully employed
